Oh dear, what a fiasco and a very dangerous situation IMHO. Watching passengers escaping through car end doors gives a real sense of the panic that they must have felt. I can think of several recommendations but after watching that three that immediately spring to mind are; (1)Electrically lock car end doors to prevent unsupervised egress. (2)Fit emergency door open buttons, active as long as a train is within station limits. (3)Install improved fault detection, smoke and fire detection systems.
Obviously interlocking would be required in order to enable car doors to be opened only at those cars in the platform along with other necessary detection systems i.e. train movement detection.
Perhaps a total rethink of operational incidents and mitigation thereof in an ever more safety conscious world! For instance an in-car passenger - driver intercom enabling instant comms to the driver in an emergency interlocked with the PEA which should always be available.

When that link is clicked on, it shows the same information. If the interim report is newer, it may be that the RAIB has updated the information on their web page at the same (original) link.

I feel that places far too much blame on the staff dealing with the situation, and not enough on the failings from management and training.
I would like to put one of those investigators in the front and see how they react with PEAs going off in 7 different cars.
Having had dealings with the RAIB in the past, I'm not exactly overconfident.

That's the trouble typical management as long as the station is open and trains are running that's all that want. Training seems to me is going down the pan as it has been over the years. RAIB my dealings with them were good but since I left things may well have changed.
